Overview:

The International Alliance Group is seeking a dedicated Health Care Social Worker to join our interdisciplinary team. In this role, you will provide support to patients and their families as they navigate complex healthcare systems. You will assess needs, provide counseling, and connect clients with essential resources to promote their well-being.

 

Key Responsibilities:

· Conduct psychosocial assessments to identify patients’ needs and challenges related to their medical conditions.

· Develop and implement individualized care plans in collaboration with medical staff, patients, and their families.

· Provide counseling and emotional support to patients and families coping with illness, disability, or end-of-life issues.

· Advocate for patients’ needs and rights within the healthcare system, ensuring they receive appropriate services and resources.

· Coordinate care with healthcare providers, including physicians, nurses, and therapists, to facilitate comprehensive patient care.

· Educate patients and families about available resources, treatment options, and support services.

· Maintain accurate and confidential documentation of patient interactions and care plans in compliance with HIPAA regulations.

· Participate in discharge planning and follow-up services to ensure continuity of care.

· Stay updated on community resources, healthcare policies, and best practices in social work.

 

Qualifications:

· Master’s degree in social work (MSW) from an accredited program.

· Valid state licensure as a Social Worker (LCSW or equivalent).

· Experience in healthcare settings, preferably with patients facing chronic illnesses or disabilities.

· Strong interpersonal and communication skills, with the ability to empathize with patients and families.

· Knowledge of healthcare systems, resources, and regulations.

· Ability to work collaboratively within a multidisciplinary team.

· Crisis intervention skills and experience dealing with sensitive situations.
