Job Description: Registered Nurse Case Manager Position Summary: The Registered Nurse Case Manager (RNCM) coordinates patient care to ensure optimal outcomes and resource utilization. This role involves assessing patient needs, developing care plans, and collaborating with healthcare teams to provide comprehensive care. Key Responsibilities:
  • Patient Assessment:
  • Conduct thorough assessments of patients' medical histories, health status, and psychosocial needs.
  • Identify patients at risk for complications and prioritize their care needs.
  • Care Planning:
  • Develop and implement individualized care plans based on assessments and patient goals.
  • Collaborate with interdisciplinary teams to create effective care strategies.
  • Coordination of Care:
  • Serve as the primary point of contact for patients and their families throughout the care continuum.
  • Coordinate services across various healthcare settings, including hospitals, outpatient facilities, and community resources.
  • Patient Education:
  • Provide education and support to patients and families regarding diagnoses, treatment options, and self-management strategies.
  • Assist patients in understanding their care plans and promote adherence to medical advice.
  • Monitoring and Evaluation:
  • Regularly evaluate patient progress and modify care plans as necessary.
  • Document all interactions, assessments, and care interventions in patient records.
  • Advocacy:
  • Advocate for patients’ needs and preferences within the healthcare system.
  • Ensure that patients receive appropriate resources and services.
  • Quality Improvement:
  • Participate in quality improvement initiatives and case reviews.
  • Contribute to the development of best practices and protocols to enhance patient care. Qualifications:
  • Current RN license in the state of practice.
  • Bachelor’s degree in Nursing (BSN) preferred; may consider an associate degree with relevant experience.
  • Certification in case management (CCM or ACM) is a plus.
  • Previous experience in case management, nursing, or a related field is preferred. Skills:
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively within an interdisciplinary team.
  • Proficient in electronic health record (EHR) systems and documentation practices. Work Environment:
  • Primarily office-based, with potential visits to patient homes, clinics, or hospitals.
